Procedures

(1) Delivery of the request form
Upon your informing the Inquiry Desk [Please click here] that you would like to request disclosure in accordance with the Personal Information Protection Act, we will send you a dedicated request form by mail.

(2) Submission of the request form
Fill out the required information on the aforementioned request form, enclose the required documents, and mail it to the address given above. Write in red on the envelope: "Retained personal data related request form included."

(3) Verification of identity
When making a request, enclose the following documents ("identity verification documents") so that we can confirm that request is being made by the person to whom the personal information pertains or by his or her representative. The identity verification documents you or your representative send will be returned at the same time that we send the reply document. However, we ask that, if there are any items displayed on the identity verification documents that are not necessary for identity verification, please mask them (e.g., blot them out) as appropriate before sending them.

(a) Requests you make for yourself
Copy of official documents, such as driver’s license, basic resident registration card with photo, individual number card (front side only), passport, or employee health insurance card or certificate

(b) Requests a representative makes on your behalf

(1) In the case of a legal representative (for a minor or adult ward)

• Documents certifying statutory agent qualification, such as a certified copy or an abridged copy of a family register, family court certificate, or certificate of registered information
• Identity verification documents for the representative himself/herself (items listed in (a) above)

(2) In the case of a delegated representative

• Power of attorney
• Identity verification documents for the representative himself/herself (items listed in (a) above)
• Identity verification documents for yourself (items listed in (a) above)

(4) Reply
We will send a reply to the person who made the request. We will endeavor to respond promptly, but it may take us about a month to reply following receipt of the request. Please note that we may not be able to comply with the request, such as if we are unable to disclose the requested retained personal data.

(5) Reply method
Our reply will be either in the form of a written document or an electromagnetic record (USB flash drive sent by mail). When making your request, specify which method you would like us to use in our reply. As a general rule, we will reply using the method you specify, but please note we may not be able to comply with your request if it is difficult to respond using that method.
Unless you specify otherwise at the time of your request, we will reply using a method that we deem appropriate.

(6) Handling fee
Regarding requests for disclosure made in accordance with the Personal Information Protection Act, a prescribed handling fee will be charged before we reply. Enclose the handling fee in the form of a postal money order with your request. Please note that we are unable to refund the handling fee even if we are unable to respond to your request, or if, after investigating, we do not have the information you requested.
Handling fee: 1,000 yen (tax included) per disclosure request
Note: You may be charged an additional amount equivalent to any costs incurred when special work or media are required, such as for creating audio transcriptions, or when we deem it necessary due to extraordinary factors, such as when we receive a large number of requests.
